Medicaid Service Coordination

Medicaid Service Coordination (MSC) is a program designed to assist people with Development Disabilities and their families in gaining access to services and support appropriate to their needs. Jewish Union Foundation’s MSC program is facilitated by qualified service coordinators and uses a person-centered planning process in developing, implementing and maintaining Individualized Service Plans (ISPs) tailored to meet the needs of each individual. Medicaid Service Coordination is provided through the Office for People with Developmental Disabilities and is available to individuals who have acquired funding through Medicaid Waiver.
